This Confined Space Entry Permit Holder is made of a .050 rigid plastic and holds a pad of 25 carbonless entry permits.  It is the law with OSHA ruling 1910.146 (c) (2) that exposed employees must be informed of the existence and the location of any danger through the posting of danger signs or by other equally effective means.  The permit provided in this kit must be completed and authorized prior to entry of any confined space area.
